To avoid a $10 per day penalty, the completed form must be postmarked by the last day of the fourth month after the licensee’s fiscal year end.

Certificate of Insurance on Grain in Licensed Grain Warehouses (PDF: 29kb)

Letter of Credit (PDF: 27kb)
An irrevocable, assignable, unconditional letter of credit may be provided as warehouse license or grain dealer license security in lieu of a bond. The letter of credit must be completed by the issuing bank. Once the letter of credit is accepted by the department, the amount may be changed on the letter of credit amendment form below.
