One Cent Apple Assessment Form #3 (PDF: 37kb)
This form may be used in conjunction with Apple Assessment Report forms 1 & 2. If you choose not to estimate your crop and split your assessment into 3 payments, you MAY pay the entire fee at once. This form may be used by itself for that purpose. You must still complete and send the 1 ½ cent Assessment form. This form is due no later than December 31 each year.

1 ½ cent Apple Assessment Form (PDF: 38kb)
This forum must be submitted in addition to the one cent apple assessment report. It is due by December 31 each year.

Beef Monthly Remittance Report Form (PDF: 129kb)
This form is for use by Missouri cattle dealers, markets, auctions and order buyers to report cattle marketed and checkoff collected on cattle sales.

Beef Private Treaty Report Form (PDF: 40kb)
This form is to be used if you sell cattle in farm-to-farm transactions. This also includes dispersal sales, production sales, breed association sales and any other sale that does not involve normal marketing channels.

Corn Monthly Remittance Report Form (PDF: 24kb)
All licensed grain dealers, trucking companies, seed producers who sell seed to other growers and retail seed dealers who buy corn seed are required to complete this form every month.

Corn Assessment Refund Application (PDF: 36kb)
For use by producers who wish to apply for a refund of corn assessments collected from them by grain elevators, grain dealers or CCC offices.

Fish Food Assessment Report Form (PDF: 36kb)
All commercial fish producers (identified for legal purposes as any producer of fish or fish products who purchases at least 2,000 pounds of fish food in a calendar year) must complete this form on a quarterly basis.

Rice Assessment Report Form (PDF: 38kb)
This form is used by licensed grain dealers who buy rice from producers.

Rice Assessment Refund Application (PDF: 36kb)
For use by producers who wish to apply for a refund of rice assessments collected from them by grain elevators, grain dealers or CCC offices.

Sheep Assessment Report Form (PDF: 66kb)
This form is used by livestock markets, auctions, fairs and festivals to report checkoff collected on sales of sheep.

Sheep Assessment Refund Application (PDF: 63kb)
For use by producers who wish to apply for a refund of sheep assessments collected from them at the time of sale.

Soybean Monthly Remittance Report Form (PDF: 18kb)
This form is used by grain elevators, licensed grain dealers, and truckers who purchase soybeans from producers to report checkoff collected.
